Instructions for Change of Address and Transcript Request

Change of Address

Please contact the Registrar's Office at 312.908.6786 with your new address. Thanks for helping us keep our records up to date!

Transcript Request

Your request must be in writing with an original signature and a payment of $5.00 per transcript. In order to complete your request, we need the following information:

1. Name at time of attendance
2. Year of graduation
3. Social security number
4. Current address
5. Address where the transcript is to be sent

Send this information to: Registrar's Office, 645 N Michigan Ave Suite 1100, Chicago, IL 60611.

Do not contact the Registrar's Office in Evanston, if you do they will forward your request and it will delay the issuance of your transcript.
